COVID Vaccines Available!
ALL LOCAL 77 MEMBERS ARE ELIGIBLE!
Dear Members,
We are thrilled to announce that ALL Local 77 members will now be eligible for a COVID vaccine through our partnership with Centennial Pharmacy.
Vaccination Dates
April 18, 2021 – April 30, 2021
- LOCATION: Centennial Pharmacy, 1020 N Delaware Ave.
- DATES (first dose): Monday, April 18th to Friday, April 30th
- DATES (second dose): Monday, May 9th to Friday, May 21st
- Time: Weekdays from 10am – 3pm, Sundays from 9am – 2pm
Day of Documentation:
- Please present your ID and insurance card with prescription insurance information
- If you are over 65 years old, please present your Medicare ID Card (red white and blue card)
- If you are on Medicare Social Security or Disability Benefits, please present your Medicare Health Insurance Card
- If you are uninsured, please present a state-issued driver’s license/ID and/or Social Security Card
Photocopies of the above information will be taken on-site, prior to your vaccination. For an expedited wait time, please bring a photocopy of the above information with you.
Included below is a pre-screening and consent form that must be completed the day that you receive your vaccine. For expedited wait time, please print the form and complete it the day of your vaccine, prior to coming to your appointment. Please remember to write you name and DOB on the top line. Please note:
- No one under the age of 18 will be vaccinated
- Individuals who are not pre-registered will NOT be able to receive a vaccine, please do not bring additional individuals who have not registered with you
- If it is discovered on the day of vaccination that someone does not qualify for a vaccine (Philadelphia is in Phase 1A and 1B) they will not receive a vaccine
- If multiple people have registered under the same email, you may only receive 1 confirmation email – that is OK
- Please wear a short-sleeved shirt for easy access to your arm
- For expedited wait time bring with you: A photocopy of your ID, insurance information (with prescription coverage information) and/or Medicare Card and your completed screening and consent form
